Dr. Boldin was born April 8th, 1960 in Gary, Indiana. Dr. Boldin attended Indiana University School of Dentistry where he graduated with a degree of Doctor of Dental Surgery. In September of 1990, Dr. Boldin opened his solo practice in the city of Valparaiso on Campbell Street. In March of 2000, the practice moved into a new state-of-the-art facility on Marquette Street where he now practices with his business partner William Lyzak, DMD, MS.Dr. Boldin performs general dentistry and has a special interest in TMJ disorders, bite alignment, complex treatment planning and full mouth reconstructive dentistry.
